Evaluation

This year we are running a prize draw for evaluations submitted.

There will be a personal prize (not relating to No Smoking Day) for the lucky winner.

Please remember to fill in your name, unit address & tel number or e-mail.

Why do we need your evaluation?

Both No Smoking Day and the Armed Forces Smoking Cessation Services need to have information about how well the campaign went this year.
The information will be useful to assess the success of this year's campaign and events and comments and suggestions will be considered for future campaigns.
Do you have any suggestions to improve the Military No Smoking Day and make it easier for you to run?
It would be sensible to keep notes as you go along during your campaign, and take photos of your displays and events. All the information you provide will be anonymous in the final report.

When writing your report, things to think about include:

Your name, Unit address, telephone number, email address.
Who was involved? List who was in your co-ordinating group.
What did you do?
Provide a brief description of any displays, events, activities or competitions you organised & attach any photos you care to submit.
Who took part?
Estimate the numbers of personnel and/or civilians who:
- Looked at your displays.
- Asked for advice
- Actively took part in No Smoking Day activities etc.
- Entered competitions etc.
What resources did you use?
List all the materials and other resources you used:
- No Smoking Day items (provided to you in the packs + any additional No Smoking Day items).
- Locally sourced items.
- Home made items.
Please say what help your Unit offered to smokers who want to stop?
- Leaflets.
- Information about helplines.
- One-to-one advice on the day.
- Information / contacts for military stop smoking services.
- Other (please describe).
