No Smoking Day and the Armed Forces Smoking Cessation Services want to improve your No Smoking Day experience and rely on your feedback and comments to make lasting improvements on the annual campaign. So if you've got a burning issue you wish to raise, remember to note it down so you don't forget. All the information will be handled with the utmost confidentiality.

Feedback questions

Include brief answers to the following questions in your evaluation:

  1. Did you receive your NSD pack(s)?
  2. Did you receive enough packs to cover the number of personnel accessing your facility?
  3. Did the pack(s) arrive in good time and intact. i.e. not damaged?
  4. Military No Smoking Day website:
    • Was it easy to access?
    • Were the contents useful?
    • Did you find the web-links helpful?
    • Do you have any suggestion on improving the website for next year.
  5. Pack Contents:
    Posters:
    • Are there enough posters in the pack?
    • Were the posters displayed around your unit.
    • If yes where.
    • If no please state limitations.
  6. Leaflets / cards:
    • Is the current issue an appropriate number?
    • If no please suggest an alternative number.
    • Are they a useful resource? If no why not?
    • Were they distributed or made available for collection? (Please be specific).

    • Any suggestions for pack contents for next year.
